Specialty Care Coordination

Our staff in the Specialty Care Coordination department can help you coordinate your care outside of HealthPartners Central Minnesota Clinics. They will assist you in making clinical appointments to the appropriate providers and make sure that the appropriate forms and materials are there for your appointment.
