Welcome to HealthPartners Central Minnesota Clinics. HealthPartners Central Minnesota Clinics have been serving the St. Cloud area, providing the best in care and service, for over 30 years. We value each of our patients and will work hard to respond to your individual needs.

Prevention and wellness are our unique focus. In addition to treating illnesses, our health care providers work with you to keep you healthy through health education and health maintenance activities. Our providers listen to your needs and become partners in reaching your health care goals.

We offer a unique range of health care services under one roof. In addition to our primary care clinic you'll find: Behavioral Health Center, Dental Center, Lab and Imaging, Pharmacy, and NovaCare Rehabilitation.

Healthy Kids Club during your medical or dental appointment your children are welcome in our Healthy Kids Club, a unique supervised play area.

To expedite your New Patient paperwork, please fill out the attached Patient Registration form, print it and bring in at your first visit. Please allow 1 business day for this to take effect.
